A big reason was the pig-headedness and arrogance of the leaders and organizers. A refusal to learn from their 'inferiors' incl their own countrymen who manned the Hudson Bay posts.

"Ice Blink," by Scott Cookman, suggests that the men of the Franklin Expedition died because a government contractor cut corners and didn't heat up the canned goods he produced for the expedition enough to prevent the development of botulism. It's an interesting and intriguing book.
